Annual Maintenance Plan

"The following document is an agreement between {{client.fullName}} (The customer) {{Job ID}} at service address: {{client.clientAddress}} and Premier Overhead (Company) to retain garage door services for one (1) year under the following terms and conditions:

1. The company will come back for a semi annual maintenance for free under this agreement, and will provide a 30 point inspectionthat includes:

Torsion Bar, Cable (Left), Cable (Right), Center Bearing Plate, Bearing Plate (Left), Bearing Plate (Right), Anchor Plate (Left), Anchor Plate (Right), Springs, Drums, Vertical Rail, J Rail, Rollers (Left), Rollers (Right), Hinges #1, Hinges #2, Hinges #3, Hinges #4, Top Roller Bracket, Screws, Opener, Sensor, Wiring, Belt / Chain/ Screw, Rail, Wall Button, Remote Transmitter, Travel Adjustment (Up), Travel Adjustment (Down), and Emergency Release Feature

1. The company will discount all parts and materials used for any future service calls under this agreement at 10% off the original price.
2. The company will discount all labor for any future service calls under this agreement at 20% off the original price.

1. The company will not charge the customer a service call for scheduling a non-emergency work order.

9. The customer agrees to pay a monthly fee of $9.95 in the beginning of every month for the next 12 months.

1. The agreement will automatically renew on a month to month base. Cancellation must be made at least 15 days before the next billing cycle.
2. Once the agreement is terminated all service call prices on labor and material will go back to the original price.
